Certification - How to Apply
The qualifications of individuals desiring certification will be reviewed by the Credentials Committee and must be approved before the candidates may take the certifying examination.

This is a protected file that uses MS Word form fields. If you do not have MS Word you can obtain a hard copy of this document from the Executive director, Dr. Mel Balk.
Forms must be accompanied by the examination fee as established by the Board of Directors. Should an applicant's credentials or qualifications not meet the prerequisites for taking the certifying examination, a portion of the application fee will be returned.
Fees
A $200 nonrefundable application fee must accompany your application.
If your application is approved, you will be notified by mail. At that time you will be asked to submit a $500 examination fee.
Payments can be made by check, VISA, MasterCard, and AMEX.

Each candidate must meet two specific requirements before taking the certifying examination.
